Pay by credit card using our secure server. We accept Visa, MasterCard, Discover and American Express, via our secure server.

Or, if you who feel uncomfortable sending credit card information through the Internet, the following methods are available:

• Order by Telephone: Our telephone order center is open from 9 am until 5 pm Central Standard Time. In the US and Canada, use our toll free number 1-800-xxx-xxxx.
• Order by Fax: Print out your order form (shopping cart), add your credit card information and fax to 817-XXX-XXXX.
• Order by Mail: You may print out your order form (shopping cart) and send it with a check or money order. The check or money order must in U. S. dollars be drawn on a U. S. bank. If paying by check, please allow 14 business days for your order to ship. Paying by check slows down your order since we don't ship until your funds clear. Money Orders will ship without delay.

Credit Terms
We may extend credit terms to businesses, government agencies, school districts, hospitals, medical clinics and other established institutions. Please email info@healthhometest.com to request a credit application.
